isoanalytics pty ltd - 40885 - radionuclide system, therapeutic, stereotactic radiosurgery, application program software - the intended use of this software is to provide a pre-treatment assessment of a stereotactic radiation therapy treatment plan. prior to a patient receiving stereotactic radiation therapy, the patient is ct scanned, and then a computer-generated treatment plan is created using a commercial treatment planning system (tps) is generated for the patient's anatomy. the treatment that is planned is then delivered using a linear accelerator that generates radiation which is directed at the tumor to destroy it. in order to verify that a patient's treatment can be delivered accurately and according to what was prescribed in the tps, patient-specific qa (psqa) is performed prior to a patient actually arriving and receiving their treatment. psqa can be performed using electronic portal imagers (epids) which are detectors mounted below the beamline of the linac which is sensitive to radiation and can therefore be used to calculate a projected dose distribution. other methods include measurement in "phantoms" that are stand-ins for the patient using a variety of radiation-sensitive detectors. the goal of psqa is to determine whether the dose planned by the tps is actually delivered accurately by the linear accelerator. we can't test this on a patient obviously and so other methods prior to the patient receiving the treatment are needed. the software presented here, efilmqa, allows the user to analyze radiochromic film (a radiation-sensitive film that changes color when exposed to radiation www.gafchromic.com) that is irradiated according to the patient's stereotactic treatment prescription and then scanned using a flatbed photo-scanner. the image of the film has varying pixel values corresponding to the optical density following scanning which is a linear function of the dose received by the sheet of film. efilmqa can compare this image to an image generated by the tps, if the two images when overlaid (registered) are similar according to well-established metrics (see: low da, harms wb, mutic s, purdy ja. a technique for the quantitative evaluation of dose distributions. med phys. 1998 may;25(5):656-61. doi: 10.1118/1.598248. pmid: 9608475.) then the psqa is determined to pass the evaluation and the treatment plan is deemed to be accurately delivered by the linear accelerator. a report is then generated which can be scrutinized and checked by medical physicists and subsequently uploaded to the patient's medical record. the general functionality of the efilmqa software is as follows: a. create calibration curves to convert pixel values in a tiff image to dose values in gray. b. create calibration curves as a function of time post-irradiation. c. read and convert .tiff image files to dose in gy using red or green channel. d. linearly scale images using known dose calibrates scanned simultaneously with the measurement film. e. manipulate 2d dose arrays geometrically in terms of rotation, flipping, cropping. f. geometrically localize the isocentre of a 2d dose distribution using markings (fiducials). g. remove markings and other ?non-dosimetric? features of a 2d image / dose distribution h. read and convert dicom and other treatment planning system (tps) dose plane files. i. register and compare 2d images / dose distributions (film / film, epid / epid, film / tps, epid, tps, tps / tps) j. perform epid mlc qa by comparing daily / monthly images to a reference (baseline) image. k. evaluate how well these 2d arrays match one another geometrically and dosimetrically using gamma analysis, difference metrics and spatially differentiated versions of these. l. create reports and figures for recording results.
